Consent needed for collecting Prescriptions, sick notes and letters Posted on 3 Jul 2018

 

Due to new General Data Protection Regulations from 1st September 2018, the Practice will no longer be able to release prescriptions, sick notes and letters to a third party without consent (over the age of 13 years). (This does not apply if you have nominated a pharmacy)

 

In order for the surgery to release your personal data to another party on your behalf, we must have your written consent.

 

By completing a consent form you are giving Manor House Surgery permission to release your prescription/Sick note or letter to a nominated third party.

 

This consent will be permanent and will be reviewed on a yearly basis.

 

 

The consent forms are available from the reception and this website.

 

Hand in completed forms to reception or post back to the practice.
